Prior to scheduling your appointment we recommend that you confirm that we are still participating with your insurance provider. We accept payments by Visa, MasterCard or Cash. We do not accept personal checks.

Please be aware that ancillary healthcare providers, such as Physician Assistants, may not be directly contracted with your commercial health plan.
