This fee schedule is for the cost of services provided by Dr. Dungy or Erin Miller, PA-C. If your insurance has out-of-network benefits, you may be able to submit charges to your insurer for full or partial reimbursement.
If you have insurance coverage that we are out-of-network with but want Dr. Dungy and Erin to perform your surgery, their surgical fees are listed below. Your insurance typically still covers the other more expensive services associated with surgery per your specific plan’s details (facility, anesthesia, physical therapy, home health). This is only the fee for the surgeon and surgical assistant.

Includes fees for the surgeon, the surgery center, anesthesia, and implants. Also, part of this package is 2 nursing visits to your home during the first 24-hours after surgery to assess you and administer additional medications; at-home physical therapy up to 3 times per week for 2 weeks; outpatient physical therapy up to 3 times per week for 4 weeks (must conclude by 8th week after surgery), and 2 post-operative visits which include x-rays.
MRI, treatment of complications, and other testing is not included.
